QUESTIONS AND INFORMATION

What will you need on your first visit?

A doctor's prescription stating ‘OT evaluation and possible treatment’ can be faxed to 582-4148 or bring it with you to your child’s first appointment.

Please bring any relevant medical, school, or therapy reports.

Release of Medical Records
and, if applicable
Sensory Processing Disorder Checklist

Make sure your child is comfortably dressed and is wearing sneakers.

What are Pediatric Potentials’ hours of operation?

Hours of operation are 9am-5pm, Monday through Thursday.

Availability for phone conference, consultation, or meetings is on Tuesday and Friday

How long is a therapy session and what does it entail?

Each therapy session is an hour including 45 minutes or an hour.

Therapy sessions are catered to the client’s goals. Treatment is creative and fun, and ranges from cooking, to road safety skills, to handwriting, to movement based activities.
